Clearly Piggy is not intelligent enough to understand that kidneys have nothing to do with testicles. But then the conclusion of his story The Ferryman's Wheelchair, showed his utter lack of knowledge in terms of anatomy.

To quote the dungeoneer "If head and neck are separated, you have been decapitated."
